Schedule Your Care Assessment

Let’s discuss your loved one’s care needs and create a support plan that fits your family, schedule, and budget.

During the assessment, we will review the type of care needed, preferred schedule, location of care, safety concerns, and whether support is needed at home, in assisted living, memory care, independent living, or a skilled nursing facility.

A $100 care assessment deposit is required to begin the care setup process. If services are started, the deposit will be applied toward the first invoice. If services are not started, a $45 administrative setup fee may be retained, and the remaining balance may be refunded according to Care Now’s payment policy.

Final rates are based on care needs, level of support, schedule, location, and caregiver availability.

Care Assessment Deposit

$100.00

A $100 care assessment deposit is required to schedule your care assessment and begin the care setup process.

If you choose to begin services with Care Now Private Duty Care, the full $100 deposit will be applied toward your first service payment.

If you decide not to move forward with services, $45 will be retained as the care assessment and administrative setup fee, and the remaining $55 may be refunded.

Final service rates are based on care needs, schedule, location, level of support, and caregiver availability.
